Foci of brown fat may be found in the mediastinum, retroperitoneum and axillae of some normal adults. Unlike other adipogenic tumors, hibernoma contains abundant mitochondria and is highly metabolically active and therefore, unlike conventional lipoma, ��hot�� or AZD3759 in vivo ��glucose-avid�� on FDG CT�CPET. Metastases, however, have never been documented. On ultrasound (US), hibernoma is typically described as a well-defined echoic mass. On magnetic resonance imaging (MRI), the lesion usually demonstrates signal intensity intermediate between that of skeletal muscle and subcutaneous fat on both T1-weighted and T2-weighted images, and exhibits contrast enhancement owing to its hypervascularization. At CT correlation, brown fat uptake corresponds to a density of ?50 to ?150 Hounsfield Units (HU). The physiologic similarities between brown fat and malignancy often lead to brown fat mimicking malignancy on FDG CT�CPET. This can SIS3 nmr easily cause diagnostic uncertainty, particularly when brown fat presents in a similar pattern as nodal uptake as in our case. Moreover, hibernomas demonstrate uptake values similar to those described for liposarcomas, and thus biopsy is required to exclude a malignant etiology. Accurate assessment is crucial because misinterpretation could lead to inappropriate therapeutic approaches like unnecessary invasive staging and diagnostic procedures or follow-up imaging with additional radiation exposure. Although hibernoma would not normally be subject to CT�CPET scanning, in the setting of a known malignancy and routine oncologic follow-up, it can lead to a false-positive diagnosis of metastasis.
