In the study, we applied a descriptive design, based on a 12-month retrospective evaluation of data (January to December, 2012). Electronic records of all patients admitted to a ward or step-down unit were considered eligible for the study. Critical care episodes were excluded because the nursing documentation was not computer-based. The electronic charts from three large metropolitan tertiary centres (> 500 beds), three urban university facilities (200�C500 beds) Resminostat and two community hospitals (100�C200 beds) were studied.
